NRL Match Head Injury Assessment Form

The NRL has a new Head Injury Assessment form which is used in the competition and must be sent to the NRL Chief Medical Officer via football operations.

It is forwarded by the club doctor WITH the completed SCAT3 for ALL HIAs (Head Injury Assessments)

All concussions need the NRL head injury form and SCAT3 performed and sent in.
